N675 Population Management and Care Transitions
Students will examine, within ambulatory and transitions of care settings, influences of community environment, public policies, and fiscal constraints relating to patient and family self-empowerment, optimal clinical outcomes, and high quality care delivery.
Course Objectives
By the end of this course, you should be able to:
- Explore evidenced based nursing interventions for increasingly complex post discharge care, patient and family centered decision making, and care coordination among diverse populations.
- Describe heath care evaluation methods across the ambulatory care environments using new technologies, informatics and public reporting to inform health care consumers’ decision making for current and future treatment.
- Explain how pathways of care post discharge, including facilitators and barriers for optimal quality and safety performance, affect health care outcomes.
- Assess quality, safety and performance improvement processes in the ambulatory environment related to reimbursement, case management and patient experience and satisfaction with care.
- Synthesize approaches to care coordination and transitions across multiple settings.
